If you wish to affiliate with one or more organizations, please type any letter in the ‘Primary Organization Name’ box and wait a few seconds for the alphabetized list of organizations to generate. You may select an organization or choose ‘Other’ located at the top of the list to input a new organization. Once you have assigned a primary organization, you may repeat this process for any of the remaining 9 organization affiliations.
